Two of the most common questions we hear at Dr L’Art in London are: “Should I have dermal fillers or biostimulators?” and “What’s the difference between them?”

Both are injectable aesthetic treatments that can address signs of ageing, volume loss and changes in skin quality. However, they work in very different ways. Understanding how each treatment works can help you have a more informed conversation with your practitioner about which approach may be suitable for your goals.

What Are Dermal Fillers?

Dermal fillers are injectable treatments designed to restore or add volume beneath the skin. Many commonly used dermal fillers are made from hyaluronic acid (HA), a substance naturally found in the body.

When injected, hyaluronic acid fillers can:

  • Restore lost volume

  • Improve the appearance of facial hollows

  • Enhance or define certain facial features

  • Support areas affected by age-related volume loss

  • Improve skin hydration in the treated area

One of the main advantages of dermal fillers is that results can be seen immediately, although there may be some temporary swelling or bruising following treatment.

How long results last depends on the product used, the treatment area and individual factors, but many hyaluronic acid fillers typically last around six to eighteen months.

What Are Biostimulators?

Biostimulators work differently from traditional dermal fillers. Rather than primarily providing immediate volume, they are designed to stimulate the body's natural collagen production.

Examples of products commonly described as biostimulators include Sculptra, Radiesse, Ellansé, Lanluma, Juvelook and Lenisna. Their ingredients, mechanisms and approved uses differ, so they should not all be considered interchangeable.

The aim of biostimulatory treatments is to encourage gradual changes in:

  • Collagen production

  • Skin firmness

  • Skin density

  • Skin texture

  • Overall facial support

Unlike conventional fillers, the results are generally gradual rather than immediate. Improvements develop over the following weeks and months as collagen production is stimulated.

The duration of results varies significantly depending on the specific product, treatment area and individual response. Some biostimulatory treatments can produce effects that persist for a year or longer.

Dermal Fillers vs Biostimulators: The Key Differences

Although both treatments can form part of a facial rejuvenation plan, there are several important differences.

How They Work

Dermal fillers: Add volume directly to the treated area.

Biostimulators: Encourage the body to produce new collagen, resulting in gradual changes to skin quality and structure.

When Results Appear

Dermal fillers: Results are generally visible immediately, although the final appearance becomes clearer once initial swelling settles.

Biostimulators: Results develop progressively over several weeks or months.

How Long They Last

The longevity of both treatments varies depending on the specific product, injection site and individual.

Dermal fillers: Many hyaluronic acid fillers last approximately six to eighteen months.

Biostimulators: Some products can produce results lasting a year or longer, but longevity varies considerably between products and patients.

What They Are Used For

Dermal fillers can be particularly useful when a specific area requires targeted volume or structural support, such as the cheeks, lips or certain facial hollows.

Biostimulators are generally used when the treatment objective involves broader improvements in collagen, skin quality, firmness or facial support.

Can They Be Reversed?

One important difference is reversibility.

Hyaluronic acid fillers can generally be treated with hyaluronidase to break down the filler when clinically appropriate. This does not mean every filler-related issue can simply be reversed, and treatment should always be performed by an appropriately qualified medical professional.

Biostimulatory products cannot generally be dissolved in the same way, which makes appropriate product selection, treatment planning and technique particularly important.

Which Is Right for You: Dermal Fillers or Biostimulators?

There isn't one treatment that is right for everyone. The most appropriate option depends on factors including your facial anatomy, skin quality, degree of volume loss and the type of result you want to achieve.

If your priority is targeted volume restoration or definition, dermal fillers may be considered. They can be particularly useful when a specific area needs additional volume or structural support.

If your priority is gradual improvement in skin quality, firmness and collagen support, a biostimulatory treatment may be considered instead.

In some cases, the two approaches can be incorporated into the same overall treatment plan, with each serving a different purpose.

Can Dermal Fillers and Biostimulators Be Used Together?

Yes. Depending on the individual and the products being used, dermal fillers and biostimulators may be combined as part of a broader treatment plan.

For example, a practitioner may use a filler to provide targeted volume while using a biostimulatory treatment to address skin quality and collagen production.

However, combining injectable treatments isn't appropriate for everyone. The products, treatment areas, timing and sequencing should be determined by a suitably qualified medical practitioner following an assessment.

Which Lasts Longer: Fillers or Biostimulators?

There is no single answer because different products have different expected durations.

Many hyaluronic acid fillers last approximately six to eighteen months, while some biostimulatory treatments can produce results that persist for a year or longer.

Rather than choosing a treatment solely because it may last longer, it is important to consider the specific product, treatment goal, expected result and individual suitability.

Do Biostimulators Look More Natural Than Fillers?

Not necessarily.

Because biostimulators generally produce gradual changes by stimulating collagen, their results may develop subtly over time. However, both fillers and biostimulators can produce natural-looking results when appropriately selected and administered by an experienced practitioner.

The amount of product used, injection technique, treatment plan and individual anatomy all influence the final result.

Is One Treatment More Painful Than the Other?

Both treatments are generally well tolerated, although the experience can vary depending on the product, injection technique and treatment area.

Topical or local anaesthetic may be used where appropriate to improve comfort. Some treatments may also contain or be administered with an anaesthetic.

Your practitioner should explain what to expect before treatment, including possible swelling, bruising, tenderness and other temporary effects.

How Do I Decide Which Treatment Is Suitable for Me?
